How to Use the Provider Search

Search Our National Network

Evergreen Health partners with PHCS, a national network of doctors. Users can click on the green text box labeled with white copy that states Go to Partner Network to gain access to the PHCS website to begin their search.

Please note, if you are searching for a Provider by County, do not use the word “county” in the search. For example, if you are searching for a Provider in “Baltimore County” type in “Baltimore” in the County search box.

Search Doctors in Maryland

Evergreen Health has its own statewide network of doctors, accessible with any Evergreen Health plan. To search for a Provider in Maryland (including Maryland doctors in the PHCS network), use the second set of search functions on the Provider Search page.

  • Doctor/Medical Group Use this text box at the center of the screen to search for doctors by name or medical group.
    You can use multiple filters to narrow your search results. However, you can make only one selection from any given filter. Make your selection based on information that you find important, and then click Search. You can narrow your results by specialty, zip code, language, gender, accepting new patients, hospital affiliation, or tier.
  • Specialty/Facility Type – This filter returns only specialists in the category you select.
  • Zip Code – This filter returns Providers/Facilities within the geographic area you choose, sorting doctors by distance from the zip code that is input into the text area.
  • Distance – When a user enters a zip code this filter returns Providers within the radius you select, sorting doctors by distance from your zip code.
  • Language – This filter returns Providers that support the language you select.
  • Gender – This filter returns only the Providers of the gender you choose.
  • Accepting New Patients – This filter returns only the Providers who are listed in the directory as accepting new patients.
  • Additional Language – This filter returns providers that support the language you select.
  • Hospital Affiliation – This filter returns Providers affiliated with the hospital you select.
  • Tier (Individual Plans Only) – This filter returns Providers that are available in either Tier 1 or Tier 2 of the directory. You can save money by receiving your care through a Tier 1 Provider. Tier 1 Providers include Evergreen Health Primary Care Offices. Note: A Network Provider’s Tier relates only to your cost share for Covered Services, and does not imply that a Physician, Health Care Practitioner, or health care Facility is more or less qualified than another. Not all categories of Providers have Tier 1 Network Providers.

Search Doctors in Maryland

How do I delete filters?
After you have begun your search, you can reset filters by clicking Reset Search in the Search Area.

Search Results
Results are shown for all Providers that meet your selected criteria. Use the Sort By function in the top right corner above the results to sort alphabetically by name, address, or Tier.

Sort
By default, your results are sorted alphabetically. If you chose the Distance From You filter, your results are sorted by distance, with the closest Providers listed first. You can change the sort from Alphabetical or Address or Tier by clicking the Sort by link above your search results.

How do I start a new search?
To start a new search, enter your criteria in the search box at the top of the screen.

Provider Contact Information
Each individual listing in your search results includes the Provider’s contact information. Additionally, if the Provider has more than one address, you can access the More Locations link to see a list of the Provider’s locations, for any individual office.

Provider Detail: You can learn more about a Provider and get a map to the Provider’s location by clicking on the Provider or Facility name. Except where noted, all information about this physician’s name, gender, hospital affiliation, office location, languages, office hours, and acceptance of new patients is self-reported by the physician and only updated upon his or her request. The accuracy of this information is validated at least annually.

Phone
On the detail page a user is able to click on the Provider/Facility telephone number on a mobile device to dial the Provider/Facility directly.

Print
Clicking the Print link at the top of the Provider detail sends your results list to the default printer defined for your computer. Your printed list includes the same information viewable on the search results page.

Email
Clicking the Email link at the top of the Provider detail sends you to your email client. This allows you to email a link to the Provider/Facility details page.

Map
Here you can map the Provider address and get driving directions. A map is available in each detailed Provider profile. Clicking the Provider link opens a new page displaying the Provider address, map, link to directions, and the ability to print and email directions. You can enter a starting address to have the route mapped for you. The directions are listed on the next page, with the complete route highlighted on the map. You can zoom in or out on the map, and toggle between views.

Can’t Find the Doctor you are looking for?
If the user is unable to find their doctor or Provider in the Maryland Search they are prompted to go to search in the partner network for PHCS.

Provider Information Validation

Below you will find an explanation of the ways you may search and what Explanation of information available through the use of our provider directory.

Name/Gender/Office Location: Except where noted, all information about this physician’s name, gender, and office locations are self-reported by the physician and only updated upon his or her request.

Medical Group Affiliations: Medical Groups in Evergreen Health’s network with which the physician is affiliated. Medical Group Affiliation is self-reported by the physician and only updated upon his or her request.

Languages: Languages spoken by the physician, clinical staff, or non-clinical staff. This does not include English. Languages are self-reported by the physician and only updated upon his or her request.

Accepting New Patients: This indicates whether physicians are accepting new patients. Accepting New Patients is self-reported by the physician and only updated upon his or her request.

Hospital Affiliations: Hospitals in Evergreen Health’s network where physicians have admitting or attending privileges. This information is self-reported by the physician and verified when the physician joins the network. The accuracy of this information is validated at least every three years.

Specialty: Specialty is self-reported by the physician and verified when the physician first joins the network, by verification of the physician’s training in the specialty or his/her Board Certification status. The accuracy of this information is validated at least every three years

Additional Locations: If the provider has additional offices, the number of offices will be listed. Clicking on the More Locations link will open a list additional office addresses in the frame.

Overview: View general information on the provider such as Gender, Accepting New Patients, Board Status, Additional Languages, and Tier.

Board Certification: Board certification is voluntary. A Board certified physician, after completing residency training in their specialty, has passed an exam and met all the requirements established by their Board. This information is verified by Evergreen Health with the American Board of Medical Specialties or the American Osteopathic Association when the physician first joins the network and at least every three years thereafter.

  • Or visit Go to “Find a DO” enter the doctor’s name and “search physicians”.

Hospital Directory

Quality Data: Quality data ratings can be reviewed by accessing the following links:

Hospital Name and Address: The hospital name and address are self-reported when the hospital is contracted and credentialed. Information is updated when requested by the facility.

Accreditation: Accreditation is a process used by hospitals and health care organizations to assess their level of performance in relation to established standards and to implement ways to continuously improve. Click here to verify the hospital’s Joint Commission accreditation status. Hospital accreditation is verified by Evergreen Health when the hospital first joins the network and at least every three years thereafter.

Evergreen Health Provider Network

Evergreen Health provides a broad network of providers available to all of our members for all of our plans including Bronze, Silver, Gold and Platinum tier plans purchased on or off the Exchange by individuals or employer groups.

  • Types of practitioners in the network include but are not limited to medical doctors (MDs), doctors of osteopathy (DOs), chiropractors (DCs), doctors of podiatric medicine (DPM), psychiatrists, psychologist (PhDs), social workers, certified registered nurse practitioners (CRNPs), certified nurse midwives (CNMs), physical, speech, occupational therapists.
  • Evergreen Health selects practitioners and hospitals for its network to ensure that services are geographically accessible and are distributed so that no member residing in the service area must travel an unreasonable distance to obtain covered services.
  • Evergreen Health does not use quality measures, member experience measures or cost-related measures to select practitioners or hospitals for participation in its network
